Radiologic Technology BAS Application

Prior to completing this application, be sure to review the Radiologic Technology BAS Program Information.

You will be asked to submit the following documents along with your online application, or to the Radiologic Technology Office. Please submit your attachments as PDF or Word files only.

1. Unofficial Transcripts
   Copies of your unofficial transcripts.
   • Any/all previous college education.
   • Proof of high school graduation or official results of high school equivalency (applicants with a recognized higher education degree are exempt).
2. ARRT Registry
   Active ARRT or equivalent registry.
